docs: say how the verification treats refusals, column types, File Scan and
whole workflows

A run where one path raises and the other answers now fails, and a run both
paths should refuse is listed in variantsBothRefuse rather than withheld.
PendingFix is gone, so a variantsNotRun row names only why a run cannot be
set up, and only a knownIssues reason becomes a test's name. Each
single-column knob also runs once per type its rule admits. File Scan runs
as a transform fed the shared table's own file, so knownIssues keeps only
URL Fetcher among the sources. WorkflowParitySpec compares whole plans with
the engine.
