Patch 003.

in sync with trunk, with more on the locking, including trying to keep flush() 
involved here.

I think I'm going to split that out into its own code, and have an initial 
patch here purely of spec & tests, leaving the S3A changes to one side. Why? 
That lock stuff is complex and needs review on its own...and that shouldn't 
hold up having our syncable/output stream spec written down.
