* checkstyle of patch 005
* pull bind() method of Stub DT manager above static methods.
* change how TokenAccessProviderException inits init cause, so no need to
change signature of AzureBlobkFileSystemException (==100% linking consistency
across library versions)
* AbfsHttpOperation lists headers on DEBUG log level only. Guess who has been
having "Server failed to authenticate the request" responses.
I've also started to write the docs which are so sadly lacking on this
connector (what are namespaces? Why so many auth options? and how to configure
them?). That'll be coming separately
