Patch 011: address review comments
* {{TestCustomOauthTokenProvider}} now {{AbstractAbfsTestWithTimeout}}; there's
no expectation in test setup for credentials, and should now be working as a
Unit test.
* reread and reworked abfs doc
* address comments about dt fetcher comments &c
Testing: azure cardiff. non-scale tests happy. Running the scale tests and the
tests of ITestAzureBlobFileSystemE2E timed out...recurrent issue for us
long-haul testers.
Note (did have to tweak hadoop project pom so the mr-client-app would build;
will be filing separate JIRA for that)
