For reference, here is the patch using a ReadWriteLock
I'm not a big fan of this because it makes the swap take a long time (and
causes a big queue backup in the process). In addition, it adds locking
complexity to the Server class that makes it more complex, which was my
motivation for refactoring this type of logic into CallQueueBase in the first
place.
