I thought the &sqrt; entity was allowed since it is an HTML 4.0 entity.
A quick check showed I also used &alpha; &delta; &mu; &pi; &plusmn;
&middot; and &times;

No, could be I made a mistake on the &sqrt;  HTML 4.0 entities should
be fine.  I am OK with reverting that change if it works on browsers
without MathML support and it is just failing for me locally with
Firefox / Linux.
