BinaryHeap.remove(Object) seems to break heap order

There is nothing obviously wrong with the iterator remove() and simple
add/remove tests work fine.  The random test above fails sporadically, always
for elements which have had copies removed.  There is usually just one element
out of order (out of 100-110).  Very strange.  I will work on this.
