I am not convinced that #3 remains an issue. I’m don’t accept your premise that 
additional fixing is needed here.

Likewise, it’s not clear to me that #4 is actually occurring. I have not seen 
anything that goes substantially against the mailing list opinion go to last 
call. If you believe this has occurred, please cite an example.

If the process is working, wouldn’t that necessarily depend on the outcome of 
the discussion on the list? While it should be possible to answer the first 
portion of your question (and as I understand the process, the answer must 
surely be “Yes, unless the authors withdraw them”), the answer to the latter 
half almost certainly cannot yet be determined without undermining the process.
