Are you doing your flashing as root? (on Ubuntu, that would be using 'sudo' before any dfu-util command)? I seem to recall that if I didn't use sudo, I got that last error message. If so, it probably means that your normal user doesn't have access to all the needed parts of the usb subsystem on the machine wher you run dfu-util. I always use sudo (I'm on Ubuntu).
