You can easily swap two alsa statefiles during call in QtMoko. There are two radio buttons in menu during call "Handset" and "Speakerphone". The first one refers to /usr/share/openmoko/scenarios/gsmhandset.state and the second one is i think gsmspeakerout.state. So adjust gsmspeakerout.state e.g. so that it's the louder version of gsmhandset.state (very simple in Neocontrol) and you can switch during call between those two.
