It connects to the Apple 5.25" PC Drive, which only works with two cards:
this card and a NuBus version of it.  If you don't have anything but the
card itself, you're probably missing the daughtercard that has a 37-pin
D-shell connector on it, which is the interface to the drive itself.  The
daughtercard sits in the SE external slot, as usual.
