I have uploaded a design at

It addresses some of the issues raised by Jeff Walther.
It's a bit of a kludge though, i.e.,soldering chip caps into a vertical
position, and the wire pins would need inspection for resoldering after
network is soldered into motherbboard.
I haven't, and will not, look up part numbers for chip caps and resistors
because where I buy them locally doesn't sell them by part number.
