If the disk won't boot any Mac and you've tried blessing the system, 
assume that the System file itself has been corrupted -- not surprising 
after 17 years. Have you tried making a Diskcopy of the floppy and 
swapping the System file for a known working version?

 From memory so please feel free to correct:
The initial letter B is used by Apple to indicate UK English. Z 
indicates International English. ZM indicates multicountry including 
International English.

The xxx-yyyy is the part number. It's utterly meaningless because even 
disks in the same set don't have sequential part numbers. Like you I 
believe that the final letter indicates a revision.
