When using disk images of the apple site, I have had to convert them
using DiskCopy on OS X first before I could Rawrite (on the PC) or dd on
a linux box.

To do this open DiskCopy on OS X and select convert image, then select
the apple disk image file and for the output file it defaults to
compressed, you need to change it to read/write.  The resultant file can
be written sector by sector to a 1.44 floppy for booting, copying the
files individually doesn't normally work for a boot floppy.

If you have any problems I could send you a bootdisk.dmg file that can
be rawritten direct.
