Does HD spin up on startup? Does HD have an Apple ROM? (If not, Apple HD SC Setup unpatched will not see it) Do other formatters (Lido 7.56, Micronet SCSI utility and so on) see the HD?
If these checks don't get you further, you may have a dead drive or one which is playing up because of "stiction". Try another HD to narrow down the problem.
