Scott, you might consider trying Microsoft's InfoPath application, though it does seem to require some jiggling. http://office.microsoft.com/en-us/infopath/HA101656341033.aspx You could also consider sending out mini versions of MS Access and importing the aggregate results into a master file. However, that is a little messy.
