I had a similar problem recently with connman + ofono recently with both
Vodafone and O2 SIMs in the UK.

The problem I was seeing is due to ofono auto-provisioning contexts from
mobile-broadband-providers.

This can fail if there are multiple providers configured for the same
MCC/MNC which there are in the XML file.

A solution is to patch ofono to allow multiple providers which will
cause provisioning to work but this may then provision an incorrect
context as certain virtual telcos take precedence over the real telcos
(e.g. ASDA mobile over Vodafone)

You could look at output from ofono to see if this is happening then
edit the serviceproviders.xml to have only the provider you need for
your MCC/MNC combination
