After a lot of messing about and patience from Terry I figured a few things.
First, where and how it uses the default_md
I am going to add a key to set that everywhere as it appears we can use sha512:
"Signature Algorithm: sha512WithRSAEncryption"
Next, to use all this you really need to update the CA as well.
I think the best route is to backup all the old certs and reinstall
