well it worked for about a week and now quit working again

I am using a promise ide-max2 card to add aditional ide devices to my
computer. The first 2 ide channels are full, so in an attempt to make it
easy to upgrade I added this card to my machine when I needed more
space.

I have a cd-rw attached to the third ide channel and I have set the
hardware address to
io= 1e8
irq=11

I have and append line that says
append="ide2=0x1e8,11 hde=ide-scsi"

this worked for about one week.

what are my options? Hardware problem? I don't think so but since I
removed windows I have no sure way to test.

here is my /var/logs/dmesg what I hink is important is separated
