You mentioned three partitions... BUT you're Missing your SWAP partitions!

the minimum required is:

/  (your root dir)
/  (your swap space)
/  (Optional your home)

and you have:
/dev/hda1  = /
 /dev/hda5 = /usr
/dev/hdb1= /home

"where is is your swap space?"
