they are only for koi8-r (and maybe a few iso8859-5).

No, I'm sorry it is too late.

They are; but they show themselves as koi8-r ones...

Indeed I should change that.

You can edit the file /usr/X11R6/lib/X11/fonts/misc/fonts.dir and change

mdk_08_koi8.pcf.gz -mdk-helvetica-medium-r-normal--8-80-75-75-p-49-koi8-r
mdk_10_koi8.pcf.gz -mdk-helvetica-medium-r-normal--10-100-75-75-p-59-koi8-r

to

mdk_08_koi8.pcf.gz -mdk-helvetica-medium-r-normal--8-80-75-75-p-49-koi8-u
mdk_10_koi8.pcf.gz -mdk-helvetica-medium-r-normal--10-100-75-75-p-59-koi8-u

but those are two small helvetica-like fonts; that won't help when fixed
fonts are requested for example.

You mean a tool to edit $HOME/.i18n file ?
Yes, that would be done at some time in the future.
