A) Please stop murdering my configuration files :p  Yes, I back them
   up, and have an RCS repository for them (the ones that matter
   anyway), but I always seem to have to restore a good number of
   files in /etc (and especially /etc/sysconfig).  Why modify my i18n
   or desktop file?

B) (And this is really an msec issue, I think) If users are ALREADY
   members of "supplementary" groups, do not remove them from those
   groups.  This just creates admin headaches.

   The biggest problems with upgrades at this point are how badly they
   clobber existing configs and the excessive ammount of time they take.
   I might even be able to rpm -Fvh * faster :)

C) lilo.conf gets all kinds of extraneous garbage added to it :(
   I expect the linux_old (and can accept the failsafe_old) entry.
   But, duplicating (backing up the orig with with _old suffixes)
   non-linux partition entries creates a headache at upgrade time,
   deleting the dos_old, diag_old, etc shouldn't be neccessary since
   those sections shouldn't even be touched by the Mandrake upgrader.
   I haven't done an upgrade since the clean install I switched to
   GRUB on, so I don't know if it is doing the same to
   /boot/grub/menu.lst

D) When it asks if you want to use your existing X config, really do
   it :p.  XFDrake (and whatever its counterpart is in DrakX) ignore
   the current settings in the XF86Config.

That's all I can think of atm, but you get the idea.
