This has been working OK for me:
1) Completely format a floppy with FAT16
2) Download the latest network boot image:
ftp://ftp.rpmfind.net/linux/MandrakeCooker/cooker/cooker/images/
3) Create the boot disk:
[phil@localhost phil]$ cp network.img /dev/fd0
cp: overwrite `/dev/fd0'? y
then boot from it
4) Configure TCP/IP settings
5) Configure network settings (optional)
6) Then choose an FTP site such as:
site: ftp.rpmfind.net
directory: /linux/MandrakeCooker/cooker/cooker
