I was having the same problem on 3 systems, 2 with Asus A7V
and 1 with the Abit KT7-Raid.
The cause of the problem on all 3 systems being that the AGP
slot and USB were sharing an IRQ.
When I updated X from 4.0.3 to 4.1.0 during hardware check at
boot all USB channels were deregistered and I lost all attached USB devices.
They then had to be manually started.
Updating the the lastest build, XFree 86-4.1.0-12mdk, eliminated the
problem.
