I *hate* one-time problems... the only thing I can add is that I ran into this when running rescue while / and /usr were formatted/empty... Fired up "rescue", saw the comments listing 4 commands, was curious about what "lsparts" would give vs "fdisk -l [dev]", tried it and got command not found.
