modprobe loop takes care of it.  The first question that gc always asks
when someone reports this question is "Is devfsd still running?  Did it
die somehow?"  If you still have your system can you try to repeat it?
That info could become invaluable to the install team.

It happened to me exactly once.  I reported it to gc and pixel, then I
could never get it to repeat.  I even forcibly removed the loop module
and _tried_ to get it to screw up, but it always worked correctly after
that first time.
