Well it uses build scripts and it downloads the soruce tar ball and then
procedes to build it according to the build script. When it gets done it
installs the app on the system.
Portage is just a build system like bsd-ports that has been adapted to linux.
The build scripts also do all the nice compile time options to enable all the
goodies and performance options.
