I Encountered the bug on a 
PIII 550 128 megs of ram - Via Chipset
/dev/hda -> 40 Gig Maxtor DMA
/dev/hdb -> 52X CDROM DMA
/dev/hdc -> 40 Gig Maxtor DMA 

Layout as follows -
/dev/hda1 -> Linux Software RAID (38 Gigs)
/dev/hda2 -> SWAP (1 Gig)
/dev/hdc1 -> Linux Software RAID (38 Gigs)
/dev/hdc2 -> SWAP (1 Gig)

I used the following procedure
1> Boot into the Mandrake Installer using the expert option
2> Create the partition table as shown above.Using custom drive layout (also
tried using fdisk on an existing Mandrake box)
3> created the Raid device /dev/md0 using the "Add to RAID" button.
4> attempt to format fails loading the md.o file (according to the log console
<crtl>+<alt>+<F5>

*NOTE: This procedure works on Mandrake 9.0 but not on 9.1b3

to install 9.0 with RAID 1 (/boot and /) & RAID 5 (/usr/local/games & /home 
& /home/public & /home/storage), allows me to create RAIDS and go all the way 
through installation, at boot loader installation LILO complains that it will 
only work on RAID1 (which it is), installed image to floppy to try and bypass, 
boot from floppy and get kernel panic (no init).
Exact same setup works faultlessly on 8.2.
