I have tested against 2.4.21pre4.10 and am seeing the same effect.  I disabled
usb bandwidth enforcement and have tried with both usb-uhci and uhci and am
getting the same effect.  My kernel logs don't make it through the crash but
dmesg was showing a normal usb device detection coming up.  The usb hotplug
scripts were called and then it locked up.  Inserting the audio module for usb
without the microphone being plugged in does not lock up the machine.  I will
try with the alsa audio drivers to see if those cause a lock up as well.

I have a Labtec Axis 712 usb audio headset which worked with the 2.4.19-16/24mdk
kernels using usb-uhci but worked even better with the uhci driver.  With rc1 if
I plug the headset in during system boot I can't get past loading modules (the
system locks up shortly after inserting either usb-uhci or uhci).  If I don't
plug the headset in until after the system has booted it will lock up the system
hard.  I can submit my kernel config and any contents of /proc that will be
helpful and switch between 2.4.21xxx and 2.4.19 as well to test differences.
