I guess you have somewhere the command 'set_refinement_immediate_replacement(1)' (it should be '0' to show the dialog). This may be in your .coot.py file or in the .coot-preferences directory, i.e. Preferences. I assume in the Preferences 'Question Accept Refinement?' is set to 'Yes'. The latter should be set to 'No' (this is a bug as it should be 'Yes' to show the dialog - will fix now).
