I'm all for putting this in contrib.  Let's hold the bar relatively low and 
wait until it proves itself to graduate it elsewhere.

We could record the start of the first duplicate of a block's leading key 
earlier in the file.  This might allow a relatively efficient skip to the start 
of a block of duplicate keys that cross a block boundary.  If you just store 
the distance in uncompressed bytes, you can probably sort the rest out.
