* Update documentation to match code
* Make configurations hierarchical
* Remove global REGISTRY of configurations
* Adding global default does not reload child configurations
* (Eventually) make properly thread-safe

* Allow future satisfaction of ticket 5708 (with which I agree, but Doug does
not).
