I would say the CoreMidi clock will be synched to the audio thread anyway.
I know that once you connect some kind of audio generator to the graph that
you provide a link between the two if you want to control the generator via
midi clock.

It all depends on what you want to do, but it sounds like you are unhappy
with workflow rather than implementation of midi. Most of those apps will
be using CoreMidi, I think you will find it a reliable enough place to
start.
