The Apache filter can specify CosignRequireFactor for an additional factor(s) (and other filters something similar), while the cosign.conf file has factors that can be appended to the "cookie" keyword (which applies to a given service cookie).
Are these just two different ways of requiring a factor, or are they required to be in sync, or something else?
