This line is not an indication of failure.  It only means the filter  
will try create a new login cookie and redirect a user to the login  
server.

Have you tried loading the cosigndbg.dll or cosigntrace.dll?  These  
will produce even more verbose output that will, hopefully, make it  
easier to figure out exactly where the IIS filter is misbehaving.
