Most likely, the cosign module failed to register due to a configuration issue. Apologies for the CosignModule's lack of formal logging, but you can load up a program such as dbgview http://technet.microsoft.com/en-us/sysinternals/bb896647.aspx to help diagnose where cosign is having trouble.
