You can't hear the radio at all without a headset. I have used it without a headset and the mic in the radio seems to work fine but it is impossible to hear what they are telling you without holding it up to your ear! By the way, the headset adapter cable is junk and I had to cut it up and hard wire it into the headset because it was cutting in and out. They don't seem to be able to make headset plugs in that small size which will last very long before the wire breaks inside.
