Harry,
     I didn't actually have a failure, I found the problem before it got
to 
that point thanks to someone who wrote an article in the Capers. The night
I 
read the article, I couldn't sleep wondering if I was doomed to the same 
failure. So in the wee hours of the morning I went up to my hangar and
pulled 
my prop to inspect the tapershaft flange. I have a 100W lightbulb in my 
hangar, and believe me... this crack was visable to the naked eye. The
crack 
starts at the key-way and does kind of a sine wave around the
circumference 
of the flange/hub. It goes about 120 deg. around it. Of course  I still
have 
the piece and will furnish anyone pictures upon request. Can't imagine
what 
would have happened had I not found this problem ~ please ask your A/P to 
inspect this at annual time. As to the logbooks, there is no reported
damage 
history, although I have not ruled out a possible prop strike at some
point 
in its life that wasn't reported.
