Yes, and no: Cone. Cone uses this feature in Courier-IMAP, but not as an IMAP extension. Cone speaks an entirely different, custom, protocol when it connects to Courier-IMAP, a part of which includes functionality that's analogous to XCOURIEROUTBOX.

   in an informal document so that other mail systems could implement it?

There's really nothing to define. Stuff sent to the folder defined by XCOURIEROUTBOX gets mailed, according to the headers of the message.

Although a formal standardization would certainly be welcome, I have absolutely no doubt, whatsoever, that it's not going to happen.

So, faced with the prospect of investing a large amount of effort, for something whose prospects will certainly be doomed, my lack of enthusiasm, I hope, is quite understandable.

Any kind of formal standardization of Courier's extension is not going to go anywhere until Courier has much, much, more brainshare than it does now.
