Courier-IMAP has nothing to do with mail delivery. The only thing that Courier-IMAP will do, quota-wise, is to update the quota to reflect any mail expunge from the mailbox, or, very rarely -- when new messages are added via the IMAP COPY or APPEND command (which would get rejected if the new messages would put the maildir over quota).
If a mailbox is over quota, obviously the mail delivery agent is the one that has to reject all new mail deliveries to the account, since the IMAP server has nothing to do with new mail delivery. All an IMAP server does is provide access to existing mail in existing mail folders. In your case, it's Qmail that delivers mail to the maildir. I do not know if the qmail-ldap mutation checks maildir quotas, you'll have to ask on the qmail-ldap mailing list. Vanilla qmail does not, since it's Courier that introduced the concept of maildir quotas.
Since you have not mentioned whether or not you've manually configured Qmail to use the deliverquota utility from the Courier-IMAP source base (which does not get installed by default), one will have to assume that this is not the case. You would've obviously mentioned such an important detail, if you did. Therefore, it's almost a certainty that you never had the maildir quota properly checked, when delivering new mail, all this time.
