You can backup /usr/lib/courier-imap/etc/imapd.cnf and change the values 
in the original under [ req_dn ] in the original to suit your own site. 
Have a look at those in openssl.cnf for guidance (don't know where 
Debian puts this file - use locate (1)). Then delete 
/usr/lib/courier-imap/share/imapd.pem and restart the courier-imapd 
service; a new cert will be generated with the new values. This is what 
I do for my sites.

Google for subjectAltName. Also, the OpenLDAP ML archives are full of 
references to generating certs containing subjectAltName details.
