At the moment, I know little about LDAP, but much about databases.
This suggests mySQL when switching from flat files.

I *like* flat files.

My mind changed without changing the subject line.

Reading through the PAM documentation, I didn't see what module one
would use for a database or flat file in a custom location.  Is
there a tutorial anywhere?

This will work with Courier-IMAP as well as Postfix?  I'd like to
have the same authentication for sending and receiving.

The documentation on LOGIN is poor, so that leaves PLAIN (over TLS,
to protect the password).  We eagerly await wider mail client
implementation of SMTP-AUTH.
