My problem was in authmysqlrc - it's written in it that you can divide variable name and variable value with spaces|tabs. But that was lie - you MUST divide it with ONE whitespace. So if you write "MYSQL_NAME_FIELD name" it should work. If this doesn't help - try to run mysql with --log option to have a full log. You should see there how Courier deals with MySQL.
