It's not a fluke; I'm still getting these constantly, and almost
certainly due to 'locallowercase'. It may just be coincidence, but
all of these bounces come from either AOL or Hotmail. I'm not sure
what's going on here.
It looks like what's happening is:
1. The mailing list sends the message to a Hotmail/AOL address.
2. Hotmail/AOL accepts the message.
3. Hotmail/AOL sends a message stating the problem (user over quota,
no such user, etc.) back to the address specified by the Reply-To
header. The Reply-To header specifies an address of the format:
<listname>-bounce-<msgnum>-<user>=<remotedomain>@<listdomain>
4. Courier accepts this email, and then sends a test email to the user,
this time with a Reply-To that looks like:
<listname>-bounce1-<random_string>@<listdomain>
5. Hotmail/AOL accepts this test email.
6. Hotmail/AOL sends a reply to the test email, this time to the
address shown in (4).
7. Upon receipt, Courier translates the message sent in (6) to
lowercase, then determines that there is no such address and
bounces the message to the postmaster.
8. The postmaster receives the message.

Subject: Mailing list warning.
To: [EMAIL PROTECTED]
Date: Sat, 28 Sep 2002 02:10:01 +0000
Hello, this is the Courier mailing list manager. I manage the mailing
list <mailto:[EMAIL PROTECTED]>.
There have been problems delivering messages to you from this mailing
list. Your address is not listed in the To: header of this warning
message because all mailing list messages are blind-carbon-copied.
Sometimes this can cause delivery failures because of broken mail
filters, and I want to make sure that this warning message also gets
returned as undeliverable, if this is the case here.
If this warning message is returned as undeliverable, I will then
automatically send you one more probe message. If that message is
also returned as undeliverable, your address will be automatically
removed from the mailing list.
A list of all the undeliverable messages appears below. You may
request any message to be retransmitted by sending a blank message
to <mailto:[EMAIL PROTECTED]>.
Replace N with the message number to retransmit.
Message numbers that were returned as undeliverable:

16
A copy of the most recent non-delivery report is attached.
