I have done Courier to LDAP using both direct LDAP and PAM, but it
was to an OpenLDAP server on Linux, not Windows.
Quick bug report: Mandrake Linux's "msec" does a check for
promiscuous mode on your eth card (as a check for sniffers). That check
breaks the LDAP connection, resulting in a failure to authenticate
(returning a "temporary error" from the authdaemond.ldap) for several
seconds (maybe thirty secs?). Then eventually it recovers. This sounds
like the recent bug fixed for SQL (was it MySQL or Postgres... can't
remember).
PAM recovers quite nicely from the msec scan. Since my PAM is
doing the same LDAP lookup, I just switched to PAM.

setting it up.
