Ahh, well, it's *your* responsibility to check the logs and the queue.
With my postfix systems, aided by logcheck and pflogsum.pl, I' see when
this happens and I can add the offending system to 'tls_per_site'.

In my experience, there are too few to introduce big changes (and
exceptionally all are MS Exchange...).
