Yes, unless you have GDBM installed.

The configure script checks whether GDBM or BDB is installed. GDBM is used if available, otherwise DB is used for all data files.

If you upgrade/downgrade DB, or if you switch to GDBM, you will need to rerun the configure script again. Then, after reinstalling, rebuilding all database files: makeuserdb (if applicable), makealiases, makesmtpaccess, etc...
