You can modify outgoing mail by writing a global filter, which resubmits the mail (adding a header to avoid recursion of the process) after making modifications, and which edits the original control file to cancel the old message.
Many people have talked about hacking courier to allow a filter to modify a message by triggering courier to invalidate it's memory cache and make it reload from disk... not sure if anyone has done this but seems like a nicer alternative to me.
