Put 

: mailserver.isp.com

in /etc/courier/esmtproutes.

If you need to authenticate to send mail through this server then put

mailserver.isp.com username password

into /etc/courier/esmtpauthclient.

Check out "man courier" and search for "esmtproutes" and "esmtpauthclient" for 
the full details.
