Not sure we're talking about the same thing - my understanding ot the original mail was that the problem was experienced between courier and the destination mail server.
If the destination mail server does content scanning, the response to the "." could be held pending the results of the scan. If that were to persist for long enough (and extra scanning COULD occur depending on the origin IP address, explaining the different response for the different connection mentioned) then courier could give up thinking the server is busy. That's why I suggested testing with Telnet.
